As Taiwan officially enters an aging society, the challenge of healthcare worker shortages is becoming increasingly severe, highlighting the urgent need for transformation in traditional healthcare models.

Based on long-term commitment to community healthcare, the Taipei Medical University (TMU) System has embraced the Hospital at Home (HaH) Program, promoted by the National Health Insurance Administration (NHIA) under the Ministry of Health and Welfare. By providing patients with optimal, efficient home healthcare services through innovative models, the TMU System has demonstrated notable performance in just six months, and become a home healthcare leader in Taiwan.

Wanfang hospital: Leading HaH Enrollment in Taipei
As of the end of 2024, Wanfang Hospital had enrolled 54 patients in the HaH program, ranking first in Taipei City. With an exceptional patient satisfaction rate of 97% and no negative feedback, this data represents the dedicated collaboration among the hospital’s healthcare teams. Through this program, the hospital offers a full spectrum of in-home medical services, including bedside testing for rapid diagnostics, coordinated care transitions, and specialist consultations. By delivering hospital-level emergency care in a home setting, the program eases the burden on family caregivers, reduces emergency room congestion, and optimizes inpatient bed usage, benefiting patients, families, and the broader healthcare system.

TMU Hospital: Pioneer in home healthcare to shorten duration of treatment
With extensive experience in home healthcare, TMU Hospital has focused on promoting a highly active treatment model in their HaH program. As of the end of 2024, the hospital had successfully completed 19 HaH treatments, with urinary tract infections (UTIs) being the most common condition, achieving an 85% treatment success rate. This approach has led to:

  • Shortened treatment durations, surpassing NHIA standards.
  • Reduced medical expenses through optimized care strategies.
  • Improved patient comfort, enhancing overall recovery experiences.

This model demonstrates the potential of home-based acute care, offering a promising alternative to prolonged hospital stays.

Shuang Ho Hospital: Patient-centered comprehensive care
Shuang Ho Hospital has focused on providing comprehensive care for patients with mobility limitations affected by pneumonia, UTIs, soft tissue infections, and other conditions.

Key highlights from the December 2024 report include:

  • 40 HaH treatments performed, with 37 delivered at home and 3 in institutional settings.
  • An 80% overall success rate, ensuring timely and effective treatment.

By prioritizing patient-centered care, Shuang Ho’s healthcare team reduces both physical and mental stress for patients, while also strengthening support systems for families to ensure continuity of care and emotional well-being.

Shaping the Future of Home Healthcare in Taiwan
The TMU System’s home healthcare achievements set a new benchmark for patient-centered medical innovation. This transformative approach not only expands care options but also enhances resource efficiency, improving accessibility, affordability, and quality of service.

Looking ahead, the TMU System will continue working with community healthcare networks to:

  • Further develop home healthcare service models for broader implementation.
  • Lower medical costs while maintaining high-quality care.
  • Enhance accessibility to underserved populations.
  • Address medical resource shortages through innovative healthcare solutions.

By expanding and refining home healthcare services, the TMU System is paving the way for a more sustainable and inclusive healthcare future in Taiwan.
